If you have recently tested for a Dan or Poom please fill out the form below so we can complete process your Certificate from the Kukkiwon. Then click "Submit" button at bottom to submit the information

Each student must fill out the form so parents testing with their children need to fill this form out for themselves then again for their kids etc. Or, please see your instructor for manual hard-copy form.
